Anticoagulation Therapy in Atrial Fibrillation Patients with Renal Impairment
Introduction to Anticoagulation
- The European Society of Cardiology recommends the use of direct oral anticoagulants (DOACs) with dose adjustment according to renal function, with apixaban being the preferred option for patients with moderate renal insufficiency 1, 2, 3
Evaluación del Riesgo Tromboembólico
- Anticoagulation is recommended for all male patients with atrial fibrillation and a CHA₂DS₂-VASc score ≥2, and for female patients with a score ≥3, according to the European Heart Journal 4, 5
- Oral anticoagulation should be considered in men with a CHA₂DS₂-VASc score of 1 and in women with a score of 2, taking into account individual characteristics and patient preferences 4
Selección del Anticoagulante según Función Renal
- For patients with moderate renal insufficiency (CrCl 30-49 mL/min), the European Heart Journal recommends dose reduction for rivaroxaban to 15 mg once daily 2
- Apixaban is recommended at a dose of 2.5 mg twice daily if the patient has a serum creatinine ≥1.5 mg/dL (133 μmol/L) and is ≥80 years old or weighs ≤60 kg 2
- Edoxaban is recommended at a dose of 30 mg once daily if CrCl <50 mL/min 2
- Dabigatran does not require specific dose adjustment for this category 2
Consideraciones Especiales y Precauciones
- The European Heart Journal recommends that anticoagulation should be continued for at least 4 weeks after cardioversion and long-term in patients with thromboembolic risk factors, regardless of whether sinus rhythm is achieved 1
- Ablation with a catheter is recommended in patients with paroxysmal or persistent atrial fibrillation resistant or intolerant to antiarrhythmic drugs 1
- Combinations of oral anticoagulants with platelet inhibitors should be avoided, as they increase the risk of bleeding 4
- Regular monitoring of renal function is recommended in patients with atrial fibrillation on anticoagulant therapy to allow for dose adjustment in those with DOAC 2
- In patients with high bleeding risk but need for anticoagulation, apixaban, edoxaban, or dabigatran 110 mg (where available) should be considered, as they show significantly less major bleeding compared to warfarina 3
Algoritmo de Decisión para Anticoagulación en Insuficiencia Renal
- The algorithm for deciding on anticoagulation in renal insufficiency involves evaluating renal function (CrCl) and thromboembolic risk (CHA₂DS₂-VASc) 4
- If CrCl >30 mL/min, DOACs with dose adjustment according to renal function are preferred 4
- If CrCl 15-30 mL/min, warfarin (INR 2.0-3.0) or apixaban with reduced dose should be considered 4
- If CrCl <15 mL/min or dialysis, warfarin with close monitoring should be considered 4
- Renal function should be monitored every 3-6 months in patients with moderate renal insufficiency and every 1-3 months in those with severe renal insufficiency 4
