Acute Leukemia Diagnosis and Management
Diagnostic Priorities
- The National Comprehensive Cancer Network recommends confirming the diagnosis with peripheral blood smear and bone marrow examination, including peripheral blood FISH using dual probes for BCR and ABL genes to confirm or exclude CML, in patients with suspected acute leukemia or CML in blast crisis 3
- Bone marrow cytogenetics and karyotype analysis should be performed to assess blast percentage in peripheral blood and bone marrow, with ≥15% suggesting accelerated phase or blast crisis in CML, according to the Annals of Oncology 1, 2
- Qualitative RT-PCR for BCR-ABL1 transcripts should be requested if CML is suspected, as recommended by the Annals of Oncology 1, 2
Risk Assessment and Emergency Considerations
- The Annals of Oncology recommend assessing for respiratory symptoms, neurological changes, or priapism in patients with marked leukocytosis, even if the WBC count does not meet criteria for hyperleukocytosis (>100 × 10⁹/L) 1, 2
- Baseline metabolic panel, uric acid, LDH, and phosphate should be checked to assess tumor lysis syndrome risk, as suggested by Praxis Medical Insights 5
Immediate Supportive Management
- The National Comprehensive Cancer Network recommends initiating aggressive hydration and tumor lysis prophylaxis immediately, with intravenous fluids at 2.5-3 liters/m²/day, as stated in the Journal of the National Comprehensive Cancer Network 3
- Allopurinol or rasburicase should be started for tumor lysis syndrome prophylaxis, according to Praxis Medical Insights 5
- Hydroxyurea 50-60 mg/kg/day can rapidly reduce WBC counts if needed, as recommended by the National Comprehensive Cancer Network and Praxis Medical Insights 3, 4, 5
Definitive Treatment Based on Diagnosis
- If CML is confirmed, tyrosine kinase inhibitor (TKI) therapy should be started immediately, with imatinib as first-line treatment for chronic phase CML, according to the National Comprehensive Cancer Network 3, 4
- BCR-ABL transcript levels should be measured every 3 months during treatment, and bone marrow cytogenetics should be performed at 6 and 12 months from therapy initiation, as recommended by the National Comprehensive Cancer Network 3, 4
