Rosuvastatin Dosing Guidelines

Standard Dosing by Patient Population

  • The American Heart Association recommends starting with rosuvastatin 5 mg once daily in patients of Asian descent, due to approximately 2-fold higher rosuvastatin plasma levels compared to Caucasian patients 1, 2
  • The FDA specifically recommends this lower starting dose for Asian patients, including Japanese, Chinese, Malay, and Asian Indian populations 1, 2

Intensity-Based Dosing Strategy

  • The American Diabetes Association recommends rosuvastatin 5-10 mg daily as moderate-intensity statin therapy, appropriate for primary prevention in adults aged 40-75 years with diabetes and no additional ASCVD risk factors 3
  • The American College of Cardiology recommends rosuvastatin 20-40 mg daily for high-intensity statin therapy, reserved for patients with established ASCVD, LDL-C ≥190 mg/dL, or diabetes with additional ASCVD risk factors 6, 3

Special Populations and Dose Adjustments

  • The National Kidney Foundation recommends not exceeding 10 mg daily in patients with severe renal impairment (CrCl <30 mL/min) 7
  • The American College of Cardiology recommends limiting rosuvastatin to 5 mg daily when co-administered with cyclosporine, tacrolimus, everolimus, or sirolimus due to 7-fold increase in rosuvastatin exposure 4

Common Pitfalls to Avoid

  • Failing to adjust starting dose for Asian ethnicity increases risk of myopathy and adverse effects due to higher drug exposure, as recommended by the American Heart Association 1, 2
  • Ignoring renal function requires dose limitation to prevent toxicity, as recommended by the National Kidney Foundation 7
