Optimizing Sertraline Dose and Augmentation Strategies

Dose Optimization and Augmentation

  • Allow at least 2-4 weeks at each dose level to assess response before further titration, as recommended by the American Academy of Child and Adolescent Psychiatry 1, 2

Add-On Medication Options

  • Lithium augmentation is a well-established strategy for antidepressant non-response, with a dosage of 150-300mg daily targeting blood levels of 0.2-0.6 mEq/L, according to the American Academy of Family Physicians 3
  • Atypical antipsychotic augmentation may be considered if depression includes psychotic features, as suggested by the American Academy of Family Physicians 3
  • Bupropion addition may be considered, starting with 37.5mg daily and increasing by 37.5mg every 3 days to a maximum of 150mg twice daily, as recommended by the American Academy of Family Physicians 3
  • Mirtazapine addition may be considered, starting with 7.5mg at bedtime and increasing to a maximum of 30mg, as suggested by the American Academy of Family Physicians 3

Critical Safety Considerations

  • The American Academy of Child and Adolescent Psychiatry recommends avoiding combining multiple serotonergic agents due to serotonin syndrome risk, and monitoring closely in the first 24-48 hours after any dose change 1, 4, 5

Non-Pharmacological Augmentation

  • The American Academy of Child and Adolescent Psychiatry suggests considering adding Cognitive Behavioral Therapy (CBT) to combination therapy, as it shows superior outcomes compared to medication alone, particularly for anxiety comorbidities 1, 2, 6
