Treatment of Mycoplasma Pneumonia in Children
Introduction to Treatment
- The American Academy of Pediatrics and Infectious Diseases Society of America recommend azithromycin as the first-line treatment for mycoplasma pneumonia in children, dosed at 10 mg/kg on day 1, followed by 5 mg/kg once daily on days 2-5 1, 2, 3
Specific Dosing for This Patient
- For a 28 kg child, the recommended dose is 280 mg azithromycin on day 1, then 140 mg once daily on days 2-5 1
Outpatient vs. Inpatient Decision
- The American College of Emergency Physicians suggests outpatient management is appropriate if the child has oxygen saturation >92% on room air, can tolerate oral medications, has no severe respiratory distress, and has reliable follow-up available 4, 5
- Hospitalization is indicated if oxygen saturation ≤92% on room air, severe respiratory distress, unable to maintain oral intake, or toxic appearance 4, 5
Treatment Algorithm
- For outpatient management, start oral azithromycin immediately at the specified doses 1, 2, 3
- Alternative macrolides if azithromycin is unavailable: clarithromycin 15 mg/kg/day or erythromycin 40 mg/kg/day 1, 2
- For children ≥7 years with macrolide allergy or treatment failure, consider doxycycline 2-4 mg/kg/day 1, 2, 3
Reassessment Timeline
- Evaluate clinical response at 48-72 hours after starting treatment, with signs of treatment success including defervescence, improved respiratory symptoms, and decreased work of breathing 4, 5
Management of Treatment Failure
- If no improvement or deterioration at 48-72 hours, consider switching to alternative antibiotics such as doxycycline or levofloxacin 1, 2
- Obtain chest imaging to evaluate for complications 3, 5
Supportive Care Measures
- Maintain oxygen saturation >92% with supplemental oxygen if needed, ensure adequate hydration, and use antipyretics for fever control 4
- Do not perform chest physiotherapy as it provides no benefit in pneumonia 4
