Initiation and Monitoring of Finerenone in Adults with Type 2 Diabetes and Chronic Kidney Disease

Eligibility Criteria

  • Finerenone should be added to a maximally tolerated ACE inhibitor or ARB in adults with type 2 diabetes and CKD who have an estimated GFR ≥ 25 mL/min/1.73 m², albumin‑to‑creatinine ratio ≥ 30 mg/g, and serum potassium ≤ 4.8–5.0 mmol/L before the first dose. 1
  • All three criteria (eGFR ≥ 25 mL/min/1.73 m², UACR ≥ 30 mg/g, potassium ≤ 5.0 mmol/L) must be met simultaneously; finerenone is never used as monotherapy or as a replacement for ACEi/ARB therapy. 1

Baseline Therapy Requirements

  • Patients must have been on a stable, maximally tolerated dose of an ACE inhibitor or ARB for at least one month prior to adding finerenone. 1

Dosing Algorithm According to Baseline eGFR

Baseline eGFR (mL/min/1.73 m²) Recommended Initial Dose of Finerenone
> 60 20 mg once daily
25 – 60 10 mg once daily
< 25* Do not initiate (continue only if already on therapy)

Monitoring Protocol

  • Baseline: Verify serum potassium ≤ 4.8–5.0 mmol/L, document eGFR and UACR ≥ 30 mg/g. 1
  • 4 weeks after initiation: Mandatory serum potassium measurement. 1
  • 4 weeks after any dose adjustment: Mandatory serum potassium measurement. 1
  • Ongoing: Regular serum potassium monitoring throughout therapy; periodic eGFR checks. Finerenone may be continued even if eGFR falls below 25 mL/min/1.73 m² provided potassium stays controlled. 1

Dose Adjustment Based on Serum Potassium

  • Potassium < 4.8 mmol/L: Titrate dose from 10 mg to 20 mg once daily. 1
  • Potassium ≤ 5.5 mmol/L: Maintain current dose. 1
  • Potassium > 5.5 mmol/L: Discontinue finerenone; once potassium ≤ 5.0 mmol/L, restart at 10 mg once daily. 1

Contra‑indications and Common Errors to Avoid

  • Do not use finerenone as monotherapy; it must always be combined with an ACE inhibitor or ARB. 1
  • Do not combine finerenone with other mineralocorticoid receptor antagonists (e.g., spironolactone, eplerenone). 1
  • Do not omit the 4‑week serum potassium check after initiation or any dose change. 1
  • Do not automatically stop finerenone solely because eGFR falls below 25 mL/min/1.73 m² if potassium remains within target. 1

Position Within Comprehensive Cardiorenal Care (KDIGO 2022)

  • KDIGO 2022 recommends that adults with type 2 diabetes and CKD receive an ACE inhibitor or ARB as first‑line therapy, followed by an SGLT2 inhibitor when eGFR ≥ 20 mL/min/1.73 m²; finerenone is added as an additional agent for patients who meet the eligibility criteria. 2
