Initial Treatment Approaches for Light Chain Myeloma

Risk Stratification and Treatment

  • The International Myeloma Society recommends risk stratification to guide therapy, with standard risk including t(11;14), t(6;14), hyperdiploidy 1, 3
  • The American Society of Hematology suggests that intermediate risk includes t(4;14), cytogenetic del 13, hypodiploidy, and high risk includes del 17p, t(14;16), t(14;20), high-risk gene expression profile 1, 3

Initial Treatment Based on Risk and Transplant Eligibility

  • The National Comprehensive Cancer Network recommends VRd (bortezomib, lenalidomide, dexamethasone) for 4 cycles in high-risk patients, followed by autologous stem cell transplant (ASCT) and VRd-based maintenance for at least 1 year 1, 2
  • The European Society for Medical Oncology suggests that intermediate-risk patients receive CyBorD (cyclophosphamide, bortezomib, dexamethasone) for 4 cycles, followed by ASCT and bortezomib-based maintenance for at least 1 year 1

Special Considerations

  • The American College of Cardiology is not relevant here, however, the International Myeloma Society recommends VCd (bortezomib, cyclophosphamide, dexamethasone) for patients with acute renal failure due to light-chain cast nephropathy 2, 5
  • The National Comprehensive Cancer Network suggests that bortezomib-based regimens can be administered without dose adjustment in severe renal impairment and dialysis 5

Supportive Care

  • The American Society of Hematology recommends thromboprophylaxis for patients on immunomodulatory drugs (lenalidomide), with aspirin for standard-risk patients and low-molecular weight heparin, warfarin, or direct thrombin inhibitors for high-risk patients 2
  • The International Myeloma Society suggests herpes zoster prophylaxis for all patients on proteasome inhibitors, pneumocystis jiroveci prophylaxis for patients on dexamethasone, and levofloxacin during the first two cycles for all newly diagnosed patients 2

Pitfalls to Avoid

  • The European Society for Medical Oncology recommends prompt initiation of bortezomib-based therapy in patients with renal failure, and emphasizes the importance of supportive care measures, particularly thromboprophylaxis and antimicrobial prophylaxis 2, 5
  • The National Comprehensive Cancer Network suggests that prolonged induction therapy (>4-6 cycles) in transplant-eligible patients may impair stem cell collection, and that risk stratification is crucial in treatment selection 1, 3
