Psychiatric Symptoms Lasting One Year

Definition and Diagnosis

  • Delirium is characterized by acute onset and fluctuating course, with symptoms typically resolving within days to weeks once the underlying cause is addressed, as per the Confusion Assessment Method (CAM) with 82-100% sensitivity and 89-99% specificity 1, 2
  • The American College of Physicians recommends that a one-year duration of psychiatric symptoms fundamentally contradicts the core diagnostic criteria for delirium 1, 2

Differential Diagnosis

  • Dementia is characterized by insidious onset and progressive course, and the American Academy of Neurology recommends waiting 2-4 weeks after any acute confusional state resolves before formal cognitive testing to avoid confounding 2
  • Delirium superimposed on dementia has a grave prognosis, leading to accelerated cognitive and functional decline, but the delirium component itself should have resolved, as stated by the American Geriatrics Society 1

Diagnostic Approach

  • The American Academy of Family Physicians recommends obtaining a detailed collateral history to establish the exact timeline of symptom onset, progression, and any fluctuations from family or caregivers 1, 2
  • The American College of Physicians recommends documenting baseline cognitive function to determine the patient's cognitive and functional status before symptom onset 2
  • A comprehensive medication review is necessary to identify anticholinergic burden and potentially deliriogenic agents that may contribute to chronic cognitive impairment, as suggested by the American Geriatrics Society 2

Medical Workup

  • A complete metabolic evaluation is recommended to screen for thyroid dysfunction, vitamin deficiencies, electrolyte abnormalities, renal/hepatic dysfunction, as per the Endocrine Society 2
  • Infectious workup is necessary to rule out chronic infections like neurosyphilis or HIV that can cause persistent psychiatric symptoms, as stated by the Infectious Diseases Society of America 2

Psychiatric Evaluation

  • Psychiatric consultation is essential for accurate diagnosis and treatment planning for chronic psychiatric conditions, as recommended by the American Psychiatric Association 2, 3

Treatment

  • The American Geriatrics Society recommends avoiding prolonged inappropriate use of antipsychotics, which carries significant risks including extrapyramidal symptoms, metabolic syndrome, and increased mortality in elderly patients with dementia 2, 4
