Unfractionated Heparin Dosing and Monitoring Protocol for Therapeutic Anticoagulation
Initial Dosing
- The American College of Cardiology recommends an initial bolus of 80 units/kg IV, followed by a continuous infusion of 18 units/kg/hour for therapeutic anticoagulation, with monitoring of activated partial thromboplastin time (aPTT) at 6 hours post-initiation and dose adjustments to maintain aPTT at 1.5-2.5 times normal value 1, 2
- For patients without IV access, an alternative subcutaneous regimen is 333 units/kg loading dose, followed by 250 units/kg every 12 hours 1, 2
Monitoring Protocol
- The American Heart Association recommends that the first aPTT measurement should be obtained 6 hours after the initial bolus dose, with a target aPTT range of 1.5-2.5 times the normal control value 3
- Adjust infusion rate according to aPTT results using a standardized nomogram, with the following adjustments:
Special Considerations
- The National Comprehensive Cancer Network recommends that UFH is the preferred anticoagulant for patients with severe renal insufficiency (CrCl < 30 mL/min) as it's primarily metabolized by the liver 1, 2, 4
- UFH is contraindicated in patients with heparin-induced thrombocytopenia (HIT); use alternative anticoagulants such as argatroban, danaparoid, or fondaparinux in these cases 5
Monitoring Frequency
- The American Heart Association recommends monitoring aPTT approximately every 4-6 hours until stable in therapeutic range, and then daily checks 3
- Periodically monitor platelet counts throughout therapy 5
