Treatment of Vasomotor Rhinitis

Pharmacological Treatments

  • The American Academy of Allergy, Asthma, and Immunology recommends the concomitant use of ipratropium bromide nasal spray and an intranasal corticosteroid for treating rhinorrhea, as it is more effective than either drug alone 7
  • Management of vasomotor rhinitis should be based on symptom spectrum, duration, severity, and patient preferences, according to the American Academy of Allergy, Asthma, and Immunology 8

Treatment Approach

  • The American Academy of Allergy, Asthma, and Immunology suggests that treatment should be individualized considering factors such as patient age, frequency and severity of symptoms, and response to previous medications 8

Managing Vasomotor Rhinitis Exacerbated by Weather Changes

Understanding Weather-Sensitive VMR

  • Weather-sensitive VMR is triggered by temperature changes, barometric pressure shifts, and weather variations, and is distinct from allergic rhinitis, with symptoms including nasal congestion, clear rhinorrhea, and postnasal drip triggered by cold air, heat, humidity changes, and temperature extremes 9
  • The pathophysiology of weather-sensitive VMR involves autonomic nervous system imbalance with heightened parasympathetic activity and hypersensitive nociceptive neurons 9
  • Negative allergy testing confirms the diagnosis of weather-sensitive VMR, which is a distinct subgroup of vasomotor rhinitis 10

Environmental Control Strategies

  • Aggressive avoidance of weather-related triggers is essential and should be implemented immediately, including wearing a scarf or mask over the nose during cold weather to warm and humidify inspired air 11
  • Minimizing transitions between extreme temperatures and maintaining indoor humidity at 30-50% can reduce mucosal irritation 9, 11, 12
  • Planning indoor activities during weather fronts when possible can help reduce symptoms 9, 12

Adjunctive Therapies

  • Nasal saline irrigations should be used regularly to remove irritants and reduce congestion, and can be performed before medication administration to enhance drug delivery 10
