Initial Assessment and Management of Acute Stroke in Young Patients

Diagnostic Evaluation

  • The American Heart Association recommends that patients with acute stroke undergo urgent and systematic evaluation, including laboratory tests such as complete blood count, electrolytes, glucose, renal function, coagulation, and troponin, to identify potentially treatable causes and determine eligibility for acute therapies 1, 2, 3
  • The American College of Cardiology suggests that blood glucose levels should be monitored at least four times a day for the first three days 4
  • The International Journal of Stroke advises against waiting for laboratory results to initiate neuroimaging or acute treatment, except in specific situations, such as patients on warfarin who require INR monitoring 2

Neuroimaging

  • The American Heart Association recommends that non-contrast head CT be performed immediately to exclude intracranial hemorrhage 1, 5, 6, 3
  • The International Journal of Stroke suggests that CT angiography of the aortic arch to the vertex be performed at the time of initial CT, when possible, to evaluate extracranial and intracranial circulation 3
  • The American College of Radiology recommends that CT be completed within 25 minutes of arrival, with interpretation within 45 minutes 1

Cardiovascular Evaluation

  • The American Heart Association recommends that an electrocardiogram be performed due to the high incidence of cardiac disease in patients with acute stroke 1, 2
  • The International Journal of Stroke suggests that echocardiogram be performed to investigate cardioembolic source 2

Monitoring and Prevention of Complications

  • The American Heart Association recommends that temperature be verified at least four times a day for three days, treating if >37.5°C 4, 3
  • The American College of Physicians suggests that oxygen saturation be monitored, with supplemental oxygen only if <94% 4
  • The International Journal of Stroke advises against delaying neuroimaging while waiting for laboratory results, as the therapeutic window for thrombolysis is critical (3-4.5 hours) 2
