Management of Recurrent Oral Candidiasis in Immunocompetent Patients

Initial Treatment Approach

  • The Infectious Diseases Society of America recommends treating acute episodes of oral candidiasis with fluconazole 100-200 mg daily for 7-14 days, achieving >90% response rates and superior to topical therapy 1, 2, 3
  • Alternative treatment for mild disease is clotrimazole troches 10 mg 5 times daily for 7-14 days 1

Chronic Suppressive Therapy

  • The key management decision is implementing maintenance therapy given the frequency of recurrence (3-4 episodes in 3 months qualifies as "frequent or disabling") 2, 3, 4
  • Fluconazole 100 mg three times weekly is the most convenient and well-tolerated suppressive regimen, continuing for at least 6 months 1, 5
  • This achieves control in >90% of patients, but after cessation, expect 40-50% recurrence rate 1

Evaluation for Underlying Factors

  • Investigate diabetes mellitus, the most common contributing factor, as well as HIV status with CD4 count, inhaled corticosteroid use, denture-related issues, and nutritional deficiencies 1, 2, 4, 5, 6, 7

If Suppressive Therapy Fails

  • Consider itraconazole solution 200 mg daily, responding in ~67% of fluconazole-refractory cases, or posaconazole suspension 400 mg twice daily for 3 days, then 400 mg daily 1, 5, 6

Practical Algorithm

  • Treat current episode with fluconazole 100-200 mg daily × 7-14 days, then initiate suppression immediately after cure with fluconazole 100 mg three times weekly, continuing for 6 months minimum 1, 5
  • Monitor for recurrence after stopping, and if relapses occur, resume indefinite suppression 1, 5
