Weekly Paclitaxel and Carboplatin Protocol for Triple-Negative Breast Cancer

Standard Dosing Regimens

  • The National Comprehensive Cancer Network recommends a weekly protocol of paclitaxel 80 mg/m² IV on days 1, 8, and 15 plus carboplatin AUC 2 IV on days 1, 8, and 15, cycled every 28 days for triple-negative breast cancer 1
  • The National Comprehensive Cancer Network suggests paclitaxel 80 mg/m² IV infusion on days 1, 8, and 15 for triple-negative breast cancer 1
  • The National Comprehensive Cancer Network recommends carboplatin AUC 2 IV on days 1, 8, and 15 for triple-negative breast cancer 1
  • The National Comprehensive Cancer Network recommends a cycle length of 28 days for triple-negative breast cancer 1

Non-Hematologic Toxicities

  • The National Comprehensive Cancer Network recommends monitoring patients closely for peripheral neuropathy and considering dose modifications as needed for triple-negative breast cancer 1

Critical Treatment Delivery Considerations

  • The National Comprehensive Cancer Network recommends reducing paclitaxel dose by 20% or holding until improvement to grade ≤1 for grade ≥3 peripheral neuropathy in triple-negative breast cancer 1

Common Pitfalls to Avoid

  • The National Comprehensive Cancer Network recommends using AUC 2 weekly instead of AUC 6 every 3 weeks when using weekly paclitaxel for triple-negative breast cancer to avoid excessive toxicity 1

Metastatic Setting Considerations

  • The American Society of Clinical Oncology suggests considering atezolizumab plus nab-paclitaxel or pembrolizumab plus chemotherapy for superior overall survival in PD-L1-positive triple-negative breast cancer 2
  • The American Society of Clinical Oncology recommends PARP inhibitors (olaparib or talazoparib) over chemotherapy after prior treatment in BRCA-mutated triple-negative breast cancer 2
  • The American Society of Clinical Oncology suggests sacituzumab govitecan demonstrates dramatic superiority over standard chemotherapy in triple-negative breast cancer after ≥2 prior therapies 2
  • The National Comprehensive Cancer Network recommends genetic testing for all triple-negative breast cancer patients to identify PARP inhibitor candidates 2

Carboplatin Addition to Taxane Regimens in Triple-Negative Breast Cancer

Neoadjuvant Setting

  • The addition of carboplatin to taxane-based neoadjuvant chemotherapy significantly increases pathologic complete response rates from 37% to 53% in triple-negative disease, according to the National Comprehensive Cancer Network 3

Metastatic Setting

  • For metastatic stage IV disease, the NCCN guidelines recommend albumin-bound paclitaxel plus carboplatin as a superior combination regimen for metastatic TNBC, demonstrating significantly longer progression-free survival and overall survival 4
  • The TNT trial showed that patients with germline BRCA1/2 mutations had significantly better response to carboplatin (68.0% vs 33.3%, p=0.03) 4

Essential Testing Requirements

  • All triple-negative breast cancer patients should undergo germline BRCA1/2 mutation testing to identify candidates for PARP inhibitors and to inform carboplatin benefit, as recommended by the NCCN guidelines 4

Guideline Recommendations

  • The European Society for Medical Oncology (ESMO) guidelines note that evidence for adding carboplatin to neoadjuvant regimens is mixed and does not consistently translate to improved survival outcomes, only to increased pCR rates 5
