Drug Interactions Between Modafinil and Bupropion

Introduction to Interactions

  • When combining modafinil and bupropion, clinicians should exercise caution and implement close monitoring for additive stimulant effects, cardiovascular changes, and neuropsychiatric symptoms, though no absolute contraindication exists for their concurrent use 1

Pharmacodynamic Interactions

  • Both medications affect dopamine and norepinephrine systems, creating potential for additive stimulant effects when used together 1
  • Bupropion functions as a norepinephrine-dopamine reuptake inhibitor, directly increasing synaptic availability of these neurotransmitters 2
  • The combination may produce additive effects on blood pressure and heart rate, as both medications independently cause cardiovascular stimulation 1
  • Increased risk of anxiety, agitation, and insomnia due to overlapping stimulant properties 1
  • Enhanced risk of seizures, particularly in patients with predisposing factors, as bupropion lowers seizure threshold 1

Pharmacokinetic Interactions

  • Modafinil's enzyme induction properties could theoretically reduce bupropion plasma concentrations, though this specific interaction has not been formally studied 3
  • The hepatitis C treatment guidelines note that modafinil is a moderate P-gp and CYP inducer that can reduce exposure to drugs metabolized through these pathways 3

Clinical Management Strategy

  • Before initiating combination therapy, screen for history of seizures or seizure disorders, eating disorders, uncontrolled hypertension, and closed-angle glaucoma 1
  • Start with lower doses of both medications when used concomitantly 1
  • Consider intermittent rather than continuous daily dosing of stimulants when clinically appropriate 1
  • Monitor for signs of excessive stimulation, including insomnia, anxiety, agitation, appetite suppression, unintended weight loss, blood pressure, heart rate, and emergence of seizure activity 1

Special Populations

  • Both medications cross the placenta and should be used with extreme caution during pregnancy 1
  • If treatment is necessary, use the lowest effective doses 1
  • Both drugs are present in breast milk; monitor breastfed infants for irritability, poor feeding, and sleep disturbances 1
  • Maximum daily bupropion dose should not exceed 450 mg to minimize seizure risk 5

Evidence and Pitfalls

  • No formal studies specifically examine the combination of bupropion and modafinil 1
  • Most interaction data comes from in vitro studies or small clinical trials that may not reflect real-world polypharmacy scenarios 4
  • Bupropion is contraindicated in patients with seizure disorders or eating disorders due to significantly elevated seizure risk 1
  • The combination with modafinil may theoretically increase this risk through additive CNS stimulation 1
