Management of Depression in Patients with Dementia

Introduction to Treatment Guidelines

Patients with dementia should avoid drugs with anticholinergic adverse effects, as these can worsen cognitive decline, according to the Alzheimer's and Dementia guidelines 1
The American Academy of Neurology recommends considering non-pharmacologic interventions first for depression in dementia, including cognitive behavioral therapies and physical activity, as stated in the Alzheimer's and Dementia journal 2

SSRIs are considered first-line treatments for agitation and neuropsychiatric symptoms in vascular cognitive impairment and dementia, with serotonergic antidepressants significantly improving overall neuropsychiatric symptoms, agitation, and depression in individuals with VCI, as per the Alzheimer's and Dementia guidelines 2
The American Heart Association suggests reserving SSRIs only for moderate to severe depression to balance treatment benefits against risks of increased cognitive decline and hemorrhage, as published in the Stroke journal 4
Cholinesterase inhibitors (donepezil, rivastigmine, galantamine) may be considered for vascular cognitive impairment in selected patients, improving cognitive function and ADLs in patients with vascular cognitive impairment and Alzheimer dementia, according to the Alzheimer's and Dementia and Stroke journals 4, 5

Discontinuation of antidepressants should be considered if clinically meaningful worsening of dementia has occurred over the past 6 months, no clinically meaningful benefit was observed at any time during treatment, the individual has severe or end-stage dementia, development of intolerable side effects, or medication adherence is poor, as recommended by the Alzheimer's and Dementia guidelines 6, 7
Deprescribing should occur gradually with dose reduction by 50% every 4 weeks until the initial starting dose is obtained, then discontinue after 4 weeks, as stated in the Alzheimer's and Dementia journal 7
