Management of Heterogeneous Uterine Echotexture in Reproductive-Age Women
Diagnostic Approach
- The American College of Obstetricians and Gynecologists recommends excluding pregnancy first with β-hCG testing in all reproductive-age women before proceeding with further evaluation 1
- Saline infusion sonohysterography is mandatory when transvaginal ultrasound demonstrates focal endometrial abnormality or heterogeneous echotexture, with 96-100% sensitivity and 97% accuracy in distinguishing polyps from submucosal fibroids 1
- MRI with gadolinium should be obtained when the uterus is incompletely visualized by ultrasound or findings are indeterminate, with sensitivity up to 79% for endometrial cancer 1, 2
- Neither ultrasound nor MRI can definitively distinguish benign from malignant pathology; tissue diagnosis remains mandatory 2
Medical Management
- The American Family Physician recommends levonorgestrel-releasing intrauterine system as the most effective treatment for menorrhagia 3
- Combined oral contraceptives are recommended for women requiring contraception 3
- Cyclic progestins, such as medroxyprogesterone acetate, can be used for ≥3 months 3
- NSAIDs and tranexamic acid can be used for ovulatory bleeding 1
Surgical Intervention
- Surgery is recommended when medical treatment fails, is contraindicated, or not tolerated, or when endometrial sampling shows hyperplasia or malignancy 1, 3
- Endometrial ablation can be considered for completed childbearing 1
- Hysterectomy is a definitive treatment option 3
- Morcellation is contraindicated when malignancy is suspected, as it can upstage disease and worsen outcomes 2
Critical Clinical Pitfalls
- The presence of fibroids on ultrasound does not exclude concurrent endometrial pathology, including cancer or endometrial stromal neoplasms 2
- Postmenopausal women with fibroids and bleeding must have endometrial cancer and uterine sarcoma excluded before any treatment 2
- If medical management fails, further investigation with imaging or hysteroscopy is mandatory before abandoning treatment 3
