Management of Dermoid Cysts in Young Women

Diagnostic Approach

  • Ultrasound is the definitive imaging modality for diagnosing dermoid cysts, with characteristic findings including echogenic attenuating components, small horizontal interfaces, or macroscopic fat with calcifications representing hair and teeth, as recommended by the American College of Radiology 1, 2
  • Transvaginal ultrasound combined with transabdominal ultrasound provides optimal visualization, according to the American College of Radiology 2
  • The presence of macroscopic fat with or without calcification and Rokitansky nodule is diagnostic, as stated by the American College of Radiology 3, 4, 5

Management Algorithm

  • For premenopausal women with dermoid cysts <10 cm, the American College of Obstetricians and Gynecologists recommends annual surveillance if not surgically removed, or proceeding directly to surgical removal, particularly in younger women 2
  • For dermoid cysts ≥10 cm, surgical removal is indicated due to increased technical difficulty and risk of complications, as recommended by the American College of Obstetricians and Gynecologists 2

Surgical Approach

  • Laparoscopic cystectomy is the preferred surgical approach for dermoid cysts, offering superior outcomes compared to open surgery, according to the American College of Obstetricians and Gynecologists 6, 7, 8
  • Laparoscopic surgery has advantages including shorter hospital length of stay, less postoperative pain, and shorter operative time, as stated by the American College of Obstetricians and Gynecologists 6, 7, 8

Special Considerations

  • If dermoid cyst is diagnosed during pregnancy, surgical management should not be delayed if torsion is suspected or the patient is symptomatic, as recommended by the American College of Obstetricians and Gynecologists 6, 7, 8
  • Laparoscopic surgery is safe throughout pregnancy when performed with appropriate precautions, according to the American College of Obstetricians and Gynecologists 6, 7, 8

Monitoring

  • Annual ultrasound surveillance is required for non-surgical candidates, with immediate evaluation if morphology changes or vascular components develop, as recommended by the American College of Radiology 2
