HIV Risk Assessment and Prevention for High-Risk Black MSM

Risk Stratification

  • Black males account for 43% of HIV/AIDS cases among men despite representing only 12% of the US population, according to the Centers for Disease Control and Prevention 1, 2
  • The presence of gonorrhea increases per-act HIV transmission probability substantially, as STDs facilitate HIV acquisition through mucosal inflammation, as noted by the Clinical Infectious Diseases journal 1, 4

Clinical Implications

  • The American College of Physicians recommends that MSM with recurrent bacterial STI diagnosis within the past 12 months should be offered PrEP immediately, as stated in the Annals of Internal Medicine 3, 5
  • The Centers for Disease Control and Prevention recommends doxycycline post-exposure prophylaxis for MSM who have had bacterial STI diagnosed in the past 12 months, as noted in the MMWR Recommendations and Reports 3, 6

Mandatory Prevention Interventions

  • The World Health Organization recommends HIV testing every 3 months minimum for high-risk individuals, using tests approved for acute/primary HIV-1 infection detection, as stated in the Annals of Internal Medicine 3, 5
  • The American College of Physicians recommends comprehensive STI screening at all exposure sites every 3-6 months, as noted in the Clinical Infectious Diseases journal 3
  • The Centers for Disease Control and Prevention recommends addressing the synergistic relationship between STIs and HIV through risk reduction counseling, including consistent condom use and partner notification, as stated in the MMWR Recommendations and Reports 1, 3, 6
