Dosage Adjustment of Doxycycline in Renal Insufficiency

Pharmacokinetics and Dosage

  • The American Thoracic Society recommends that doxycycline can be administered without dose reduction in patients with renal insufficiency, as its elimination is primarily biliary and not renal, with a daily dose of 100-200 mg 1, 2
  • The pharmacokinetics of minocycline, a related tetracycline, also remains stable in renal insufficiency, confirming this class characteristic for certain tetracyclines, with no significant difference in serum half-life between patients with normal renal function and those with severe renal insufficiency 3

Clinical Considerations

  • The Centers for Disease Control and Prevention suggests that, unlike streptomycin, amikacin, and kanamycin, which require a reduction in frequency to 2-3 times a week in renal insufficiency, doxycycline maintains its daily dosage, with no need for dose adjustment in patients with stable renal insufficiency 1, 2
  • The American College of Chest Physicians recommends avoiding concomitant administration of doxycycline with antacids, mineral salts, or iron, which decrease absorption, regardless of renal function, and considering discontinuation of the medication in patients with stable renal insufficiency who develop acute deterioration of renal function while taking doxycycline 4

Doxycycline Dosing in Renal Impairment

Important Clinical Caveats

  • The Kidney Disease Outcomes Quality Initiative (KDOQI) guidelines recommend avoiding tetracyclines other than doxycycline in renal impairment, as tetracyclines can exacerbate uremia and require dose reduction when GFR <45 mL/min/1.73 m² 5
