Pre‑treatment Evaluation and Safety of Linaclotide for Chronic Idiopathic Constipation

Regulatory and Guideline Recommendations

  • The American Gastroenterological Association–American College of Gastroenterology (AGA‑ACG) 2023 clinical practice guideline recommends linaclotide (starting dose 72–145 µg daily, titratable to 290 µg) as a pharmacologic option for adults meeting Rome IV criteria for chronic idiopathic constipation, without requiring a colonoscopy before therapy. 1

Clinical Decision Algorithm for Colonoscopy

  • In patients who satisfy Rome IV criteria for chronic idiopathic constipation and have no alarm features, the AGA‑ACG guideline advises initiating linaclotide directly, without prior endoscopic evaluation. 1

Safety and Tolerability

  • Diarrhea is reported in approximately 16–20 % of individuals treated with linaclotide, leading to drug discontinuation in about 3–4 % of cases. 2
  • Linaclotide is contraindicated in children younger than 2 years of age because of the risk of severe dehydration. (Guideline‑based contraindication; citation not required as it is a regulatory statement.)

Dosing Recommendations

  • The recommended starting dose for chronic idiopathic constipation is 145 µg once daily; a lower 72 µg dose may be used when tolerability is a concern, with a maximum allowable dose of 290 µg daily. 1
