Diagnostic Approach for Elderly Black Male with Left Breast Lump

Initial Assessment and Imaging

  • The American College of Radiology recommends bilateral diagnostic mammography as the initial imaging study for men 25 years and older with an indeterminate or suspicious palpable breast mass, with a sensitivity of 92-100%, specificity of 90-96%, and negative predictive value of 99-100% 1, 2
  • For men with a palpable breast mass, targeted ultrasound should be performed if mammography is negative or shows an indeterminate finding that does not clearly correlate with the palpable abnormality, with the combination of mammography and ultrasound detecting 93-100% of cancers that are occult on mammography alone 1, 2, 5, 6
  • The American College of Radiology suggests that digital breast tomosynthesis (DBT) is an acceptable alternative to standard mammography 2, 4

Biopsy and Tissue Diagnosis

  • The American College of Radiology recommends image-guided core needle biopsy as the procedure of choice for suspicious breast masses, with ultrasound guidance preferred when the lesion is visible on ultrasound, and stereotactic-guided core biopsy for lesions visible only on mammography 1, 2, 4
  • Core needle biopsy is superior to fine-needle aspiration in sensitivity, specificity, and correct histological grading, and a marker clip should be placed post-biopsy to document sampling location 4

Risk Factors and Special Considerations

  • The American College of Radiology notes that male breast cancer frequently presents at advanced stage with larger tumor size and higher probability of nodal metastases, and that black men may have different risk profiles, though specific data is limited 1, 2, 4
  • Approximately 50% of men with breast cancer may have coexisting gynecomastia, and gynecomastia is NOT a risk factor for male breast cancer, but the two conditions can coexist 1, 4

Diagnostic Algorithm

  • The American College of Radiology recommends a diagnostic algorithm that includes bilateral diagnostic mammography, targeted ultrasound, and image-guided core needle biopsy for suspicious breast masses, with consideration of risk factors and special considerations for elderly black men 1, 2, 4, 5, 6
