Seroquel Discontinuation Guidance

Introduction to Tapering

  • Medications acting on the central nervous system, including antipsychotics like quetiapine, require cautious discontinuation to prevent withdrawal effects, as recommended by the Journal of the American Geriatrics Society 1

Tapering Protocol

  • Reduce the dose by 25% of the current dose every 1-2 weeks as tolerated, for a patient on 50mg nightly, translating to a reduction schedule of approximately 37.5mg, 25mg, 12.5mg, and finally discontinuation, as suggested by Praxis Medical Insights 2

Monitoring Requirements

  • Establish weekly follow-up visits during the tapering process to assess for symptom return and withdrawal effects, monitoring specifically for withdrawal symptoms, return of original psychiatric symptoms, and somnolence changes, as recommended by Praxis Medical Insights 2

Special Considerations

  • Patients taking other CNS depressants may require even more gradual tapering, as noted by the Mayo Clinic Proceedings 3 and Praxis Medical Insights 4

Reinstituting Therapy

  • If severe psychiatric symptoms return during tapering, reinstate the previous effective dose and consult a psychiatrist before attempting further discontinuation, as advised by Praxis Medical Insights 2 and 4

Documentation and Education

  • Document the tapering plan and rationale clearly, and provide patient and family education about the difference between withdrawal symptoms and relapse of the underlying condition, as recommended by the Journal of the American Geriatrics Society 1 and Praxis Medical Insights 4
