Guanfacine (Intuniv) Dosage and Treatment Guidelines for ADHD

Dosing Recommendations

  • The American Academy of Pediatrics recommends initiating guanfacine extended release (Intuniv XR) at 1 mg once daily and titrating by 1 mg weekly based on response and tolerability to a recommended target dose range of 0.05 to 0.12 mg/kg/day or 1 to 7 mg/day 1, 2, 3
  • The maximum daily dose should not exceed 7 mg, as recommended by the American Academy of Pediatrics 1, 3

Age-Specific Recommendations

  • The American Academy of Pediatrics approves guanfacine extended release for children and adolescents aged 6-17 years, with evidence being stronger for stimulant medications in elementary school-aged children (6-11 years) 1, 4, 5, 6
  • For adolescents (12-18 years), guanfacine can be prescribed as part of ADHD treatment, according to the American Academy of Pediatrics 5, 6

Combination Therapy

  • The American Academy of Child and Adolescent Psychiatry suggests that combination therapy with stimulants may be beneficial in some cases, though this should be approached with caution, and guanfacine may be effective for children with intellectual disability and ADHD 7
