Antithrombotic Therapy After Bioprosthetic TAVR

Standard Post-TAVR Antithrombotic Strategy

  • The American College of Cardiology recommends dual antiplatelet therapy with aspirin 75-100 mg plus clopidogrel 75 mg daily for 6 months after TAVR, though this is based on clinical trial protocols rather than direct evidence of benefit 1, 3
  • The American Heart Association suggests anticoagulation with warfarin for at least 3 months may be reasonable in patients at low bleeding risk, as subclinical valve thrombosis occurs in 7-40% of TAVR patients on antiplatelet therapy alone but not in those receiving warfarin 2, 4

Long-Term Management

  • The American College of Cardiology recommends lifelong aspirin 75-100 mg daily alone for all patients with bioprosthetic valves who have no other indication for anticoagulation 1, 3, 5
  • Chronic anticoagulation is not required unless the patient has a separate indication such as atrial fibrillation, history of thromboembolism, left ventricular dysfunction, or hypercoagulable state 1, 4

Special Considerations

  • The American Heart Association notes that multidetector CT studies reveal 7-40% of TAVR patients develop reduced leaflet motion and valve thrombosis when treated with antiplatelet therapy alone, but this finding is absent in patients receiving warfarin 2
  • Direct oral anticoagulants, including dabigatran, apixaban, and rivaroxaban, should not be used in patients with any prosthetic valve due to excessive thrombotic complications 1, 3, 5

Special Populations

  • Chronic anticoagulation is required for patients with a history of thromboembolism, with warfarin and INR target 2.5 (range 2.0-3.0) indicated long-term 1, 4
  • Patients with left ventricular dysfunction require chronic anticoagulation, with warfarin and INR target 2.5 (range 2.0-3.0) indicated long-term 5
  • Patients with hypercoagulable conditions require chronic anticoagulation, with warfarin and INR target 2.5 (range 2.0-3.0) indicated long-term 1, 4
  • Patients with bioprosthetic mitral valve require chronic anticoagulation, with warfarin and INR target 2.5 (range 2.0-3.0) indicated long-term 3, 4
