Flying Precautions for Patients with Leg Swelling

Risk Assessment and Prevention

  • The American College of Chest Physicians recommends that patients with leg swelling wear properly fitted below-knee graduated compression stockings (15-30 mmHg at ankle) to reduce the risk of deep vein thrombosis (DVT) during air travel 1, 2, 3
  • Low-risk travelers with leg swelling alone have a baseline flight-related DVT risk of approximately 1 per 4,600 flights over 4 hours, and compression stockings are suggested but not mandatory 1, 3
  • High-risk travelers, including those with recent surgery, history of previous DVT or pulmonary embolism, active cancer, or known thrombophilia, should use enhanced prophylaxis measures, including compression stockings and possibly prophylactic low-molecular-weight heparin (LMWH) 1, 4

Universal Prevention Measures

  • The American Society of Hematology recommends that all travelers wear loose, comfortable clothing, increase fluid intake by 0.5-1.0 liters per day, and avoid alcohol and excessive caffeine to reduce the risk of DVT 2, 4, 5
  • Requesting an aisle seat and walking the cabin aisles every 2 hours can also reduce the risk of DVT 5, 6, 7
  • Performing calf muscle exercises, such as ankle pumps, knee extensions, and calf raises, can maintain venous flow and reduce the risk of DVT 2, 5, 6

Pharmacologic Prophylaxis

  • The American College of Chest Physicians recommends that high-risk travelers use prophylactic LMWH in weight-adjusted dosing administered 2-4 hours before departure for flights over 4 hours 1, 6
  • Aspirin is not recommended for flight-related DVT prevention due to insufficient evidence of benefit and meaningful bleeding risk 1, 3, 6

Special Populations

  • Women using oral contraceptives or hormone replacement therapy who travel by air face an 8.2-fold elevated VTE risk and should consider compression stockings and possibly prophylactic LMWH 6, 8
  • Heart failure patients should ensure stable condition before travel, continue prescribed medications, and monitor body weight if on moderate-to-high dose diuretics 2, 5
