Complications of Renal Biopsy

Major Complications

  • Intraperitoneal hemorrhage can occur in 0.032-0.7% of cases, intrahepatic hematoma and hemobilia occur in approximately 0.006% of cases, and perirenal hematomas requiring intervention are among the most common major complications, according to the American Journal of Kidney Diseases 1
  • Death directly related to renal biopsy is extremely rare, occurring in approximately 0.009-0.33% of cases, and arteriovenous fistulae can form as a complication of the biopsy procedure 1

Minor Complications

  • Microscopic hematuria is common and usually self-limiting, and gross hematuria occurs in a small percentage of patients but typically resolves without intervention 1

Risk Factors for Complications

  • Multiple needle passes (exceeding 4) during the biopsy procedure increase bleeding risk, and prolonged bleeding and partial thromboplastin times are associated with increased risk 1

Alternative Approaches for High-Risk Patients

  • Transjugular (transvenous) renal biopsy is an alternative for patients with contraindications to percutaneous biopsy, such as bleeding disorders or inability to cooperate 1

Risk Reduction Strategies

  • Patients on chronic hemodialysis should be well dialyzed prior to biopsy, and heparin should be avoided during the procedure, and ensuring patient cooperation during the procedure is essential to prevent tearing of the kidney and capsule 1

Special Considerations

  • The risk of complications in patients with diabetic kidney disease is no greater than in patients with other causes of chronic kidney disease 1
