Management of Moderate to Severe Plaque Psoriasis with Biologics
Patient Selection and Biologic Therapy
- The American Academy of Dermatology recommends initiating biologic therapy with adalimumab as first-line systemic treatment for moderate-to-severe plaque psoriasis, dosed at 80 mg initial dose, followed by 40 mg one week later, then 40 mg every 2 weeks thereafter, for patients with ≥5% body surface area or PASI ≥10 with DLQI >10 1, 2, 3
- Patients are indicated for biologics when they meet all of the following criteria: PASI ≥10 AND DLQI >10, and have failed, have contraindications to, or are intolerant of at least one conventional systemic therapy (methotrexate, cyclosporine, or PUVA) 4, 1
- For psoriatic arthritis of any severity, biologics are indicated regardless of skin disease extent 1, 2, 5
First-Line Biologic Selection Algorithm
- Adalimumab is the preferred first-line biologic, with dosing of 80 mg subcutaneous on Day 1, then 40 mg at Week 1, then 40 mg every 2 weeks, achieving PASI 75 in 71% of patients at week 16 (vs. 7% placebo) 1, 2
- Infliximab is an alternative first-line option, with dosing of 5 mg/kg IV at weeks 0, 2, 6, then every 8 weeks, achieving PASI 75 in 80% of patients, but requires addition of methotrexate to reduce immunogenicity and prevent antibody formation 4, 3, 2
- Etanercept is a third TNF-α option, with dosing of 50 mg subcutaneous twice weekly for 12 weeks, then 50 mg once weekly maintenance, achieving PASI 75 in 48-66% at week 12 4, 1, 3, 5
Site-Specific Biologic Recommendations
- The American Academy of Dermatology strongly recommends adalimumab for palmoplantar psoriasis, nail psoriasis, and psoriatic arthritis, regardless of skin disease severity, as it treats both skin and joint symptoms and inhibits radiographic joint damage (Strength A) 1, 2, 3, 5
- Etanercept is also recommended for nail psoriasis and scalp psoriasis (Strength A) 1
Combination Therapy to Augment Efficacy
- The American Academy of Dermatology recommends adding high-potency corticosteroids ± vitamin D analogues to etanercept for 12 weeks (Strength A) and to adalimumab for 16 weeks (Strength B) 1, 3, 2
- Methotrexate combinations with etanercept, adalimumab, and infliximab are recommended to reduce immunogenicity and prevent antibody formation (Strength B) 1, 2
Assessment of Treatment Response
- The British Journal of Dermatology recommends assessing treatment response at 12 weeks for etanercept, 16 weeks for adalimumab, and 10-14 weeks for infliximab 4, 1
- Treatment success is defined as achieving PASI 75 (75% improvement) or PASI 50 (50% improvement) AND DLQI improvement >5 points 4
Common Pitfalls to Avoid
- The American Academy of Dermatology recommends never using systemic corticosteroids (prednisone) for psoriasis due to poor long-term efficacy and rebound flares, and not delaying biologics in patients with >5% BSA involvement 5, 7
- Infliximab intervals should not be extended beyond 8 weeks, and methotrexate should always be added to infliximab to prevent antibody formation 6, 2
