Ertapenem Treatment Duration for Complicated Urinary Tract Infections

Treatment Recommendations

  • For complicated UTIs treated with ertapenem, the recommended duration of therapy is 5-10 days, with most patients requiring 7-10 days depending on clinical response and underlying comorbidities, as suggested by the Journal of Microbiology, Immunology and Infection 1
  • The Infectious Diseases Society of America recommends 7-10 days of ertapenem for most complicated UTIs, including acute pyelonephritis, with a strength of evidence rated as strong 1
  • For bloodstream infections from a urinary source, 7-14 days of ertapenem is recommended, according to the Journal of Microbiology, Immunology and Infection 1
  • Males with paraplegia or neurogenic bladder when prostatitis cannot be excluded should receive 14 days of ertapenem, as recommended by the Praxis Medical Insights 2, 3
  • Patients with delayed clinical response despite appropriate therapy may require 10-14 days of ertapenem, as suggested by the Praxis Medical Insights 3
  • The American Urological Association recommends considering 10-14 days of ertapenem for patients with urologic abnormalities, obstruction, or foreign bodies 2

Special Considerations

  • For males with paraplegia who are afebrile for more than 48 hours and hemodynamically stable, 7 days of ertapenem may be considered, as recommended by the Praxis Medical Insights 2
  • Rapid clinical response with fever resolution within 48-72 hours may allow for shorter courses of ertapenem, according to the Praxis Medical Insights 2
  • Male patients where prostatitis involvement cannot be clinically excluded should receive extended duration of ertapenem, as recommended by the Praxis Medical Insights 2, 3
  • Persistent fever beyond 72 hours of appropriate therapy may require extended duration of ertapenem, as suggested by the Praxis Medical Insights 3
  • The Centers for Disease Control and Prevention recommends extended duration of ertapenem for patients with complicated anatomy, such as vesicoureteral reflux, obstruction, or indwelling catheters 2
  • Immunocompromised patients or those with diabetes mellitus may require extended duration of ertapenem, as recommended by the Praxis Medical Insights 3
  • Bacteremia documented from a urinary source requires 7-14 days of ertapenem, as suggested by the Journal of Microbiology, Immunology and Infection 1
