Antipsychotic-Induced SIADH Risk

Patient Selection and Risk Stratification

  • The American Geriatrics Society identifies antipsychotics as SIADH-inducing medications, with special caution in older adults 1
  • The American College of Cardiology recommends avoiding olanzapin in risk patients due to its metabolic side effects 2, 3, 4
  • The European Society of Cardiology study shows that clozapin has an increased arrhythmia risk (OR 2.03), indicating systemic effects 5
  • Quetiapin has a documented arrhythmia risk (OR 1.29) and should be used with caution in patients with hyponatremia risk 5
  • The American College of Cardiology recommends aripiprazol as an alternative to olanzapin/clozapin due to its lower metabolic risks 2, 3, 4

Special Patient Populations

  • Older patients have an increased risk for SIADH; aripiprazol or quetiapin are preferred over typical antipsychotics 1, 6
  • For Parkinson's patients, quetiapin or aripiprazol are the only options; haloperidol is contraindicated 6, 7
