Anesthetic Management for Tendon Lengthening in Duchenne Muscular Dystrophy

Critical Contraindications

  • The American Thoracic Society recommends avoiding succinylcholine due to the risk of acute rhabdomyolysis, massive potassium release, and hyperkalemic cardiac arrest in DMD patients 3, 4
  • The use of succinylcholine can cause cardiac arrest immediately after injection, even in previously undiagnosed cases 4
  • The American Society of Anesthesiologists recommends using total intravenous anesthesia (TIVA) with propofol and short-acting opioids (remifentanil or fentanyl) for induction and maintenance 1, 2, 3
  • Non-depolarizing agents, such as rocuronium, can be used for muscle relaxation if necessary, but with caution and careful monitoring 5

Essential Perioperative Monitoring

  • The American College of Chest Physicians recommends continuous SpO₂ monitoring, blood or end-tidal CO₂ monitoring, and having an ICU bed available for postoperative care 1, 2, 3
  • Patients with FVC <50% predicted require assisted ventilation during induction, maintenance, and recovery 1, 2

Critical Preoperative Assessment

  • The American Heart Association recommends mandatory cardiology consultation before any anesthetic, as DMD causes dilated cardiomyopathy 4
  • Pulmonary evaluation should include measurement of FVC, maximum inspiratory pressure (MIP), maximum expiratory pressure (MEP), and peak cough flow (PCF) 4

Postoperative Management

  • The Society of Critical Care Medicine recommends continuous SpO₂ monitoring for a minimum of 24 hours postoperatively and using supplemental oxygen cautiously 6
  • Patients with baseline FVC <50% predicted should continue noninvasive positive pressure ventilation (NPPV) postoperatively 1, 2
