Hip Osteoarthritis Management

Introduction to Treatment Limitations

  • The American College of Rheumatology acknowledges that hip osteoarthritis cannot be reversed, with current treatments focused on symptom control, pain management, and functional improvement rather than structural restoration of damaged cartilage 1, 2, 3

Pharmacological Interventions

  • The European League Against Rheumatism (EULAR) notes that glucosamine and chondroitin lack hip-specific evidence for structural modification, despite theoretical disease-modifying potential 3
  • Diacerhein may slow joint space narrowing but does not reverse existing damage, and causes significant side effects including diarrhea (RR 3.73) 3, 4, 5
  • Avocado soybean unsaponifiables (ASU) showed no structural benefits over 2 years in the most recent trial 3, 4, 5
  • Intramuscular glycosaminoglycan peptide complex demonstrated no structural benefits versus placebo over 5 years 3

Injection Therapies

  • Hyaluronic acid has no RCT evidence supporting its use in hip OA, with only uncontrolled studies showing pain reduction from baseline 3, 4, 5

Surgical Options

  • Total hip replacement (THR) achieves 43-84% pain-free outcomes at 9.4 years follow-up, with revision rates of 0.18-2.04 per 100 person-years 1, 2, 6
  • Osteotomy may benefit younger patients with hip dysplasia or deformity, but evidence is sparse (category III) and it does not reverse OA—it redistributes mechanical forces 1, 2, 6, 7
  • Joint-preserving procedures (arthroscopic debridement, surgical dislocation) show symptom improvement but lack controlled evidence of disease modification 1, 2, 6
