Treatment of Streptococcus pneumoniae Bacteremia in Sickle Cell Disease

Initial Empiric Antibiotic Selection

  • The American College of Physicians and Infectious Diseases Society of America recommend initiating empiric treatment with IV ceftriaxone 2g daily plus vancomycin 15-20 mg/kg every 8-12 hours for S. pneumoniae bacteremia in patients with sickle cell disease, until susceptibilities confirm penicillin sensitivity 1, 2
  • The initial empiric antibiotic selection should include dual therapy with a third-generation cephalosporin, such as ceftriaxone, and vancomycin to cover potential penicillin-resistant and cephalosporin-resistant strains 1

De-escalation Based on Susceptibility Results

  • If the isolate is penicillin-susceptible, the American Heart Association recommends switching to penicillin G 2-3 million units IV every 4 hours 2
  • If the isolate is penicillin-resistant but cephalosporin-susceptible, the American Heart Association recommends continuing high-dose ceftriaxone 2g daily 2
  • If the isolate is ceftriaxone-resistant, the American Heart Association recommends continuing vancomycin and considering adding rifampin, in consultation with an infectious disease specialist 2

Treatment Duration

  • The Infectious Diseases Society of America recommends a minimum of 2 weeks of IV therapy for uncomplicated bacteremia, with criteria for uncomplicated including exclusion of endocarditis, no implanted prostheses, follow-up blood cultures at 2-4 days negative, defervescence within 72 hours, and no metastatic infection sites 3
  • For complicated bacteremia, the Infectious Diseases Society of America recommends 4-6 weeks of therapy, depending on the extent of infection 3

Critical Monitoring Requirements

  • The American College of Physicians recommends obtaining follow-up blood cultures 2-4 days after initial positive cultures to document clearance of bacteremia 3
  • The American Heart Association recommends performing echocardiography on all adult patients with bacteremia to exclude endocarditis, with transesophageal echocardiography preferred 3, 2
  • The Infectious Diseases Society of America recommends assessing for treatment failure if fever persists beyond 72 hours despite appropriate antibiotics, clinical deterioration occurs, or new complications develop 4, 1

Sickle Cell Disease-Specific Considerations

  • The Centers for Disease Control and Prevention recommend continuing penicillin prophylaxis indefinitely after recovery from pneumococcal bacteremia in patients with sickle cell disease, with standard prophylaxis being penicillin V 125 mg twice daily 5, 6
  • The Centers for Disease Control and Prevention recommend ensuring pneumococcal vaccination status is current and considering revaccination if the last dose was >5 years ago and the patient was <65 years at initial vaccination 5, 6
