Doxycycline Treatment Guidelines for Bacterial Infections

Skin and Soft Tissue Infections

  • The Infectious Diseases Society of America recommends treatment of MRSA skin infections with doxycycline 100 mg twice daily for 7-14 days based on clinical response 1, 2
  • The Clinical Infectious Diseases society suggests that treatment duration for most bacterial skin and soft tissue infections should be 7-14 days 3

Other Specific Infections

  • The Clinical Infectious Diseases society recommends doxycycline 100 mg twice daily for the treatment of tularemia, bubonic plague, and bacillary angiomatosis 3

Special Situations

  • The American Academy of Family Physicians recommends doxycycline 100 mg twice daily for 60 days for the treatment of inhalational anthrax and cutaneous anthrax 4, 5

Pediatric Dosing

  • The American Academy of Family Physicians and the Journal of Microbiology, Immunology and Infection recommend that children over 8 years and over 45 kg should receive adult dosing of doxycycline (100 mg twice daily) 4, 6
  • The American Academy of Family Physicians and the Journal of Microbiology, Immunology and Infection suggest that children over 8 years and less than or equal to 45 kg should receive 2.2 mg/kg of doxycycline every 12 hours 4, 6

Important Considerations and Precautions

  • The Journal of Microbiology, Immunology and Infection states that doxycycline is not recommended for children under 8 years of age due to potential for dental staining and bone growth effects 6, 7
  • The American Academy of Family Physicians recommends that doxycycline may be used during pregnancy for life-threatening infections, but its use should be carefully considered due to potential adverse effects on developing teeth and bones 4, 8

Doxycycline Loading Dosage Guidelines

Pediatric Dosing

  • For children, the Centers for Disease Control and Prevention recommends a loading dose of 2.2 mg/kg every 12 hours with a maximum of 100 mg per dose 9

Condition-Specific Loading Doses

  • The Centers for Disease Control and Prevention recommends a single 200 mg dose of doxycycline for post-exposure prophylaxis of sexually transmitted infections within 72 hours after sexual exposure 10
  • For inhalational anthrax, the Centers for Disease Control and Prevention recommends 100 mg twice daily for 60 days 9

Important Clinical Considerations

  • Patients should avoid lying down for 1 hour after taking doxycycline to prevent esophagitis, as recommended by the Centers for Disease Control and Prevention 10
  • The Centers for Disease Control and Prevention advises separating dosing by at least 2 hours from dairy products, antacids, and supplements containing calcium, iron, magnesium, or sodium bicarbonate 10
  • Doxycycline is generally contraindicated in pregnancy, though may be used for life-threatening infections when benefits outweigh risks, according to the Centers for Disease Control and Prevention 9
  • The Centers for Disease Control and Prevention does not recommend doxycycline for children <8 years due to dental staining risk, except when benefits outweigh risks in severe infections 9

Doxycycline Dosing Guidelines

Introduction to Doxycycline Dosing

  • The Centers for Disease Control and Prevention (CDC) recommends dosing doxycycline at 100 mg twice daily for adults and 2.2 mg/kg twice daily for children <45 kg for tickborne rickettsial diseases, with treatment continuing for at least 3 days after fever subsides and until clinical improvement, typically a minimum of 5-7 days total 11, 12
  • For anaplasmosis, the CDC recommends extending treatment to 10-14 days if concurrent Lyme disease is suspected 11, 12

Pediatric Considerations

  • The CDC states that doxycycline is safe for children ≥8 years for short courses, and for life-threatening rickettsial diseases, use in children <8 years when benefits outweigh risks—limited exposure causes negligible tooth staining 11, 12
  • Multiple studies demonstrate that short courses in children aged 6-7 years have minimal effect on permanent tooth discoloration 11

Disease-Specific Treatment

  • For community-acquired pneumonia (outpatient), the American Thoracic Society recommends 100 mg twice daily for adults without comorbidities, with some experts recommending 200 mg as the first dose to achieve adequate serum levels more rapidly 13
  • The CDC recommends that patients with suspected rickettsial diseases should not delay treatment while awaiting laboratory confirmation—delay can lead to severe disease and death 12
  • Fever should subside within 24-48 hours of treatment initiation; if not, consider alternative diagnosis 11

Administration and Safety

  • Oral therapy is appropriate for outpatients with early-stage disease who can tolerate oral intake, while intravenous therapy is indicated for severely ill hospitalized patients, particularly those who are vomiting or obtunded 11, 12

Maximum Daily Dosing of Doxycycline for Bacterial Infections

Disease-Specific Maximum Dosing

  • The Centers for Disease Control and Prevention recommends a single 200 mg dose for STI post-exposure prophylaxis, with a critical limitation of not exceeding 200 mg per 24-hour period 14, 15

General Dosing Guidelines

  • The American Academy of Pediatrics and the Infectious Diseases Society of America support the evidence that the maximum daily dose of doxycycline for adults with bacterial infections should not exceed 200 mg daily, as exceeding this dose may result in increased side effects without demonstrated additional efficacy 16, 17

Dosage and Administration of Doxycycline

Indications and Dosage

  • The Infectious Diseases Society of America recommends a single dose of 200 mg for post-exposure prophylaxis of Lyme disease, not to exceed 200 mg per 24-hour period 18, 19
  • For skin infections caused by methicillin-resistant Staphylococcus aureus (MRSA), the recommended dosage is 100 mg twice daily for 7-14 days, as suggested by the Journal of Microbiology, Immunology and Infection 20
  • The American Academy of Pediatrics and the Infectious Diseases Society of America recommend a dosage of 100 mg twice daily for children ≥8 years and ≥45 kg, similar to the adult dosage 20
  • For the treatment of certain manifestations of Lyme disease, such as peripheral or central neurological involvement, a dosage of 200-400 mg per day in divided doses may be used, as recommended by the Clinical Infectious Diseases journal 21

Special Considerations

  • The Clinical Infectious Diseases journal notes that no dosage adjustment is necessary in patients with renal insufficiency, as the pharmacokinetics of doxycycline are similar to those with normal renal function 21
