Telmisartan Contraindications and Safety Considerations

Pregnancy

  • When pregnancy is identified, telmisartan should be stopped immediately to avoid fetal renal toxicity and associated morbidity. 1

Combination Therapy

  • Telmisartan must not be co‑administered with ACE inhibitors or direct renin inhibitors because the combination increases the risk of hyper‑kalemia, symptomatic hypotension, and renal dysfunction without providing additional therapeutic benefit. [1][2]

Renal Artery Stenosis (High‑Risk Renal Situations)

  • In patients with severe bilateral renal‑artery stenosis, inhibition of the renin‑angiotensin‑aldosterone system by telmisartan can precipitate acute renal failure. [1][2]

Hyperkalemia Monitoring

  • Routine monitoring for elevated serum potassium is required in individuals with chronic kidney disease, heart failure, those receiving renal replacement therapy, or those taking potassium‑containing supplements or potassium‑sparing diuretics while on telmisartan. [1][2]

Angioedema Precautions

  • Telmisartan should be avoided in patients with a prior history of angioedema related to angiotensin‑receptor blockers; if angioedema occurred with an ACE inhibitor, a minimum wash‑out period of six weeks is recommended before initiating telmisartan. 1
