Duration of Trimethoprim-Sulfamethoxazole (TMP-SMX) for Burkholderia Eradication

Treatment Phases for Burkholderia pseudomallei (Melioidosis)

  • The recommended duration of trimethoprim-sulfamethoxazole (TMP-SMX) therapy for eradication of Burkholderia pseudomallei is 3-6 months 1
  • Treatment begins with an intensive phase of at least 14 days using intravenous antibiotics 1
  • Meropenem is an option for the intensive phase 1
  • Longer intensive treatment is required for patients with critical illness, extensive pulmonary disease, deep-seated collections or organ abscesses, osteomyelitis, septic arthritis, or neurologic melioidosis 1

Eradication Phase

  • TMP-SMX is the drug of choice for the eradication phase of melioidosis 1
  • The recommended duration for eradication phase therapy is 3-6 months 1
  • This extended duration is necessary for preventing recrudescence or later relapses of melioidosis 1

Evidence for Treatment Duration

  • A randomized trial demonstrated that TMP-SMX monotherapy for 20 weeks (5 months) had recurrence rates comparable to combination therapy 1

Alternative Regimens

  • If TMP-SMX is not tolerated or contraindicated, alternatives should also be administered for the full 3-6 month duration 1

Monitoring During Treatment

  • Regular monitoring of complete blood counts is recommended during TMP-SMX therapy 2, 3
  • Monitor for common adverse effects including rash, neutropenia, thrombocytopenia, and elevated liver enzymes 1, 4
