Hemoglobin Targets and Management in Special Pregnancy Conditions

Special Considerations for Hemoglobin Levels

  • For pregnant women with sickle cell disease, the American Society of Hematology recommends a target hemoglobin level of >7.0 g/dL, and if transfusions are needed, aim for an HbS level of <50% 1
  • In women with afibrinogenemia and hypofibrinogenemia, the International Society on Thrombosis and Haemostasis suggests a target fibrinogen level of ≥1 g/L throughout pregnancy, and increase to ≥1.5 g/L in case of vaginal bleeding or during labor and delivery 2, 3, 4
  • For pregnant women with diabetes, the American Diabetes Association recommends no specific hemoglobin targets different from non-diabetic pregnant women, but rather focuses on glycemic targets, including fasting glucose <95 mg/dL, one-hour postprandial glucose <140 mg/dL, and two-hour postprandial glucose <120 mg/dL 5, 6
