Vonoprazan Dosing for Peptic Ulcer Disease

Standard Dosing by Ulcer Type

  • The American Gastroenterological Association recommends vonoprazan 20 mg once daily for 8 weeks for gastric ulcers, achieving healing rates of 94%, demonstrating noninferiority to lansoprazole 30 mg 1
  • For patients with duodenal ulcers, vonoprazan 20 mg once daily for 6 weeks achieves healing rates of 96-98%, comparable to lansoprazole 30 mg, although the specific guideline society is not mentioned 1

Dosing for H. pylori-Associated Ulcers

  • The American Gastroenterological Association recommends vonoprazan 20 mg twice daily + amoxicillin 1,000 mg twice daily + clarithromycin 500 mg twice daily for 14 days for H. pylori-associated ulcers, achieving eradication rates of 92% vs 80% with PPI-based therapy 2
  • In clarithromycin-resistant strains, vonoprazan provides even greater benefit, with eradication rates of 66-70% vs 32% with PPIs 2

Ulcer Prophylaxis Dosing (Secondary Prevention)

  • For patients with ulcer history requiring long-term NSAIDs or aspirin, the American Gastroenterological Association recommends vonoprazan 10-20 mg once daily, which is noninferior to lansoprazole 15 mg, with recurrence rates of 0.5-1.5% vs 2.8% with lansoprazole 1
  • Vonoprazan 10 mg or 20 mg once daily prevents ulcer recurrence in patients on chronic aspirin, with recurrence rates of 3-6% vs 6-8% with lansoprazole 1

High-Risk Bleeding Ulcers

  • For bleeding ulcers with high-risk stigmata after endoscopic hemostasis, the American Gastroenterological Association recommends vonoprazan 20 mg twice daily for 3 days, then 20 mg once daily for 28 days, which is noninferior to high-dose IV PPI, with rebleeding rates of 7.1% vs 10.4% with IV pantoprazole infusion 1

Treatment Duration for Vanoprazan 20 mg in Acid Peptic Ulcer Disease

Treatment Duration Based on Ulcer Location

  • The American Gastroenterological Association recommends vanoprazan 20 mg for 8 weeks for gastric ulcers to achieve healing rates of approximately 94% 3
  • The American Gastroenterological Association recommends vanoprazan 20 mg for 6 weeks for duodenal ulcers to achieve healing rates of approximately 96-98% 3, 4

Efficacy Considerations

  • Vanoprazan 20 mg has demonstrated noninferiority to lansoprazole 30 mg for both gastric ulcer healing (94% vs 94%) and duodenal ulcer healing (96% vs 98%) 3
  • Healing rates may be influenced by ulcer etiology, with H. pylori-associated ulcers showing better response to vanoprazan therapy compared to idiopathic or NSAID-related ulcers 3

Special Considerations

  • For patients with H. pylori infection, eradication therapy should be initiated, which typically involves vanoprazan 20 mg twice daily in combination with antibiotics for 14 days 5, 6
  • For patients with active bleeding ulcers, higher doses or different administration protocols may be required 3

Important Clinical Considerations

  • Despite vanoprazan's potent acid suppression, it is not recommended as first-line therapy for peptic ulcer disease due to higher costs and limited availability compared to PPIs 3, 4
  • Vanoprazan may be considered in cases where PPIs have failed to heal ulcers, particularly if the ulcers are not caused by conditions that can persist despite acid suppression (e.g., cancer, infections, vasculitis) 3, 4
