Ropinirole Dosing Guidelines for Restless Legs Syndrome

Introduction to Ropinirole Alternatives

  • The American Academy of Sleep Medicine recommends against the standard use of ropinirole due to the high risk of augmentation, and instead suggests the use of alpha-2-delta ligands, such as gabapentin or pregabalin, as first-line therapy for restless legs syndrome, with a conditional recommendation and moderate certainty of evidence 1, 2, 3
  • The American Academy of Sleep Medicine strongly recommends gabapentin, gabapentin enacarbil, or pregabalin as first-line therapy for restless legs syndrome, with a strong recommendation and moderate certainty of evidence 1

Diagnostic Considerations

  • Before initiating pharmacological treatment for restless legs syndrome, it is recommended to check morning fasting ferritin and transferrin saturation levels, and supplement with iron if ferritin levels are ≤75 ng/mL or transferrin saturation is <20% 1
  • Augmentation occurs in a substantial proportion of patients taking ropinirole, causing symptoms to worsen, appear earlier in the day, and spread to arms or trunk, according to the Journal of the American Geriatrics Society 4, 1
  • The 2026 American Academy of Sleep Medicine guidelines represent a paradigm shift away from dopamine agonists, such as ropinirole, due to their long-term harms outweighing short-term benefits 1

Discontinuation and Alternative Therapies

  • When discontinuing ropinirole, a gradual taper over 7 days is recommended to avoid withdrawal symptoms, and consideration should be given to transitioning to gabapentin or pregabalin during the taper 1
  • Extended-release oxycodone may be needed for refractory augmentation cases when discontinuing ropinirole 1
  • For patients with end-stage renal disease on hemodialysis, gabapentin is preferred over ropinirole for restless legs syndrome, despite very low certainty of evidence, due to its lower risk of augmentation 2, 3
