Dobutamine Infusion Guidelines

Introduction to Dobutamine Dosing

  • The American College of Cardiology recommends starting dobutamine infusion at 2-3 mcg/kg/min and titrating up to a typical therapeutic range of 2-20 mcg/kg/min based on clinical response 1, 2

Standard Dosing Algorithm

  • Initial dose should start at 0.5-1.0 mcg/kg/min for cautious initiation, or 2-3 mcg/kg/min for standard initiation, without a loading dose 1
  • The dose should be increased every 15 minutes based on clinical response 1
  • The dose should be doubled at each titration step according to response or tolerability 1

Context-Specific Dosing

  • For stress echocardiography, the American Heart Association recommends starting at 5 mcg/kg/min and increasing in 5 mcg/kg/min increments every 5 minutes, with a maximum dose of 20 mcg/kg/min 3, 4, 5
  • For acute heart failure/cardiogenic shock, the American College of Cardiology recommends starting at 2-3 mcg/kg/min and titrating up to 15-20 mcg/kg/min based on perfusion endpoints 1, 2

Critical Monitoring Parameters

  • Continuous monitoring of ECG telemetry for arrhythmias, blood pressure, and heart rate is required 1, 2
  • Monitoring of signs of perfusion, such as skin temperature, mental status, and lactate, is also recommended 2

Important Clinical Caveats

  • Tolerance to dobutamine develops after 24-48 hours of continuous infusion, resulting in partial loss of hemodynamic effects 2
  • A weaning strategy of decreasing the dose by 2 mcg/kg/min every other day while optimizing oral vasodilator therapy is recommended 1, 2
  • Hemodynamic effects of dobutamine are dose-dependent, with mild arterial vasodilation at 2-3 mcg/kg/min, predominant inotropic effects at 3-5 mcg/kg/min, and both inotropic effects and potential vasoconstriction at >5 mcg/kg/min 1, 2
