Progesterone Therapy in Patients with Coronary Artery Disease

General Considerations

  • The American College of Cardiology recommends discontinuation of hormone replacement therapy (HRT) in women who experience cardiovascular events while on HRT, unless there are compelling reasons to continue 1
  • Hormone replacement therapy (HRT) containing estrogen and progesterone should not be used for secondary prevention of coronary events in patients with established coronary artery disease 2
  • If a patient develops an acute coronary event while on HRT, it is prudent to discontinue the therapy 2

Evidence Against Progesterone Use in CAD

  • The American Heart Association indicates that HRT should not be used for secondary prevention of coronary events in postmenopausal women 1

Alternative Management Approaches

  • Non-hormonal pharmacological options should be considered first-line therapy for managing menopausal symptoms in women with coronary artery disease 2
  • For genitourinary symptoms, locally applied vaginal estrogen may be considered as there is minimal systemic absorption 2
  • If severe vasomotor symptoms significantly impact quality of life and cannot be managed with non-hormonal treatments, consider consultation with both cardiovascular and menopause specialists 2
  • Use the lowest effective dose for the shortest possible time if hormone replacement therapy is necessary 2

Special Considerations

  • Women with coronary artery disease should be managed with evidence-based therapies including lipid-lowering agents 2
  • Women with coronary artery disease should be managed with evidence-based therapies including antiplatelet therapy 2
  • Women with coronary artery disease should be managed with evidence-based therapies including beta-blockers 2
  • Women with coronary artery disease should be managed with evidence-based therapies including ACE inhibitors 2
  • Lifestyle modifications remain cornerstone therapy, including smoking cessation 2
  • Lifestyle modifications remain cornerstone therapy, including regular physical activity 2
  • Lifestyle modifications remain cornerstone therapy, including optimal weight management 2
  • Lifestyle modifications remain cornerstone therapy, including heart-healthy diet 2

Monitoring and Follow-up

  • If progesterone therapy is deemed necessary despite coronary artery disease, careful monitoring for cardiovascular symptoms is essential 1, 2
  • For women with antiphospholipid antibodies or syndrome, HRT should be avoided due to increased thrombotic risk 1
