Initial Management of Ogilvie's Syndrome
Diagnostic Approach
- Obtain CT abdomen with oral contrast to exclude mechanical obstruction before diagnosing Ogilvie's syndrome, ensuring adequate hydration 1
- Discontinue anticholinergic drugs, which can reduce peristalsis, as part of the initial management 2
- Cyclizine should be discontinued immediately due to its potential to exacerbate Ogilvie's syndrome 3
Pharmacologic Management
- Neostigmine administration is contraindicated in patients with bradycardia, active bronchospasm, and mechanical obstruction, highlighting the need for careful patient selection 1
- Metoclopramide may be beneficial in incomplete obstruction but should be avoided in complete obstruction, according to guidelines from the National Comprehensive Cancer Network 4, 5
- Proton pump inhibitors can be used to reduce gastric hypersecretion, particularly in patients who have been NPO, as recommended by the Journal of the National Comprehensive Cancer Network and Clinical Gastroenterology and Hepatology 4, 6
Preventive Measures and Avoiding Pitfalls
- The American Gastroenterological Association and Clinical Gastroenterology and Hepatology recommend avoiding secretory laxatives, such as polyethylene glycol in high doses, early in treatment, as they can worsen hypokalemia and exacerbate distension 6
- Discontinuing opioids or anticholinergics is crucial, as these medications can directly worsen Ogilvie's syndrome, according to the Gut journal 2
