Management of Hyperlipidemia and Related Conditions

Assessment and Diagnosis

  • The patient presents with severe hyperlipidemia, vitamin D deficiency, and GERD, with laboratory findings suggesting possible familial hypercholesterolemia given the extremely elevated LDL level 1, 2, 3
  • Elevated hematocrit and hemoglobin levels should be monitored 4

Management of Hyperlipidemia

  • The European Society of Cardiology recommends continuing rosuvastatin 20 mg as the primary lipid-lowering therapy, with a target LDL-C goal of <1.4 mmol/L (55 mg/dL) and at least 50% reduction from baseline 1
  • If LDL-C goal is not achieved after 8-12 weeks on maximum tolerated rosuvastatin dose, add ezetimibe 1, 3
  • Monitor lipid levels 8 (±4) weeks after starting treatment and after any adjustment, and annually once target levels are achieved 5

Management of Vitamin D Deficiency

  • The American College of Cardiology recommends initiating vitamin D supplementation to correct deficiency, targeting levels >30 ng/mL 4
  • Vitamin D deficiency may increase risk of statin-associated muscle symptoms 4

Lifestyle Modifications

  • The American Heart Association recommends implementing a heart-healthy diet, limiting daily fat intake to <30% of calories, with <7% from saturated fat, and limiting sodium intake to ≤1,500 mg/day 6
  • Regular physical activity for 30-60 minutes daily at moderate intensity, and weight management to achieve healthy waist circumference (≤40 inches for men) are recommended 6
  • Consider using the OARS method for behavior change counseling 7

Follow-up and Special Considerations

  • Recheck lipid panel in 8 weeks to assess response to therapy, and monitor for statin-related adverse effects, particularly given the vitamin D deficiency 4, 5
  • Evaluate for familial hypercholesterolemia given the extremely elevated LDL-C level, and consider referral to lipid specialist for potential PCSK9 inhibitor therapy if LDL-C goals are not achieved 1, 3
  • For patients with familial hypercholesterolemia, family cascade screening is recommended 3
