Immediate Management of Anaphylaxis with Respiratory Distress

Primary Interventions

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends administering intramuscular epinephrine 0.01 mg/kg (1:1000 concentration) immediately into the anterolateral thigh, with a maximum dose of 0.5 mg in adults and 0.3 mg in children, as the first-line treatment with no absolute contraindications 1, 2, 3
  • Administer 100% oxygen immediately to all patients with respiratory distress, as recommended by the American Academy of Otolaryngology-Head and Neck Surgery 1, 3, 4
  • Establish large-bore IV access immediately, as recommended by the Anaesthesia journal 4, 6
  • Administer rapid IV fluid boluses with normal saline or lactated Ringer's solution, as recommended by the American Academy of Otolaryngology-Head and Neck Surgery 1, 3, 4, 6

Secondary Interventions

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends that antihistamines and corticosteroids should not be given before or instead of epinephrine, as they have no role in acute anaphylaxis management due to slow onset of action 1, 3
  • Consider IV aminophylline or magnesium sulfate for severe, refractory bronchospasm, as recommended by the Anaesthesia journal 4, 6

Observation and Monitoring

  • Observe patients for at least 4 hours after symptom resolution, and extend to 24 hours for severe reactions or those requiring >1 dose of epinephrine, as recommended by the American Academy of Otolaryngology-Head and Neck Surgery and the Annals of Oncology 1, 3, 5, 7
  • Obtain serum mast cell tryptase levels if diagnosis is unclear, as recommended by the Anaesthesia journal 4, 6

Post-Acute Management

  • Prescribe epinephrine auto-injector for home use with detailed instructions, as recommended by the American Academy of Otolaryngology-Head and Neck Surgery and the Circulation journal 1, 2, 3
