Gentamicin Dosage and Administration for UTI
Dosing Recommendations
- The European Association of Urology recommends administering gentamicin 5 mg/kg IV/IM once daily for 5-7 days for uncomplicated pyelonephritis requiring hospitalization 1
- For pediatric febrile UTIs, the American Academy of Pediatrics recommends 7.5 mg/kg/day divided every 8 hours for 7-14 days total therapy 2, 3
- Parenteral therapy should continue until clinical improvement (typically 24-48 hours), then switch to oral agents, as recommended by the American Academy of Pediatrics 2, 3
Special Considerations
- The American College of Clinical Pharmacy recommends that dosing in obese patients should be based on lean body mass, not actual body weight, to minimize the risk of nephrotoxicity 4
- Increasing gentamicin dose beyond recommended amounts does not enhance efficacy but increases nephrotoxicity risk, according to the American College of Clinical Pharmacy 4, 5
- The Infectious Diseases Society of America recommends that local resistance patterns should guide empirical selection of antibiotics, including gentamicin 1
Renal Impairment Adjustments
- The National Kidney Foundation recommends that standard dosing applies ONLY to patients with normal renal function, and dose reduction and extended intervals are mandatory for patients with creatinine clearance <50 mL/min 4
- The National Kidney Foundation also recommends that short-course regimens should NOT be used if creatinine clearance <20 mL/min 4
