Droperidol Dosing for Cannabinoid Hyperemesis Syndrome

  • If symptoms persist after initial dose, titrate cautiously up to 2.5 mg IV maximum, according to Praxis Medical Insights 1
  • Onset of action occurs within 3-10 minutes with duration of effect lasting 2-4 hours, as reported by Praxis Medical Insights 1, 2
  • Administer as intermittent boluses rather than continuous infusion, as recommended by Praxis Medical Insights 2

Critical Safety Considerations

  • Screen for QTc prolongation and cardiac risk factors before administration, as advised by Praxis Medical Insights 2
  • Absolute contraindications include QTc >440 ms (males) or >450 ms (females), and known history of torsades de pointes, according to Praxis Medical Insights 2
  • Relative contraindications requiring caution include congestive heart failure, bradycardia, cardiac hypertrophy, hypokalemia or hypomagnesemia, concurrent diuretic use, age ≥65 years, and alcohol abuse, as noted by Praxis Medical Insights 2

Clinical Context and Alternative Agents

  • The American Gastroenterological Association guidelines mention haloperidol among agents supported by case series for CHS, but droperidol has demonstrated superior outcomes in head-to-head comparisons 3, 4
  • If droperidol is contraindicated or unavailable, consider alternative agents such as haloperidol, benzodiazepines, topical capsaicin 0.1% cream, or olanzapine, as suggested by the American Gastroenterological Association 3, 4
  • Avoid opioids entirely as they worsen nausea and carry high addiction risk in this population, according to the American Gastroenterological Association 3

Practical Implementation

  • Obtain baseline ECG if feasible before first dose, as recommended by Praxis Medical Insights 2
  • Correct electrolyte abnormalities (potassium, magnesium) prior to administration, as advised by Praxis Medical Insights 2
  • Reduce dosage in patients with renal or hepatic dysfunction, according to Praxis Medical Insights 1
