Polyhist Forte D Dosage and Usage Guidelines

Introduction to Polyhist Forte D

  • The American Academy of Allergy, Asthma, and Immunology recommends Polyhist Forte D (diphenhydramine and pseudoephedrine) for adults with allergic rhinitis or cold symptoms, with a dosage of 25-50 mg of diphenhydramine and 60-120 mg of pseudoephedrine every 4-6 hours, not exceeding 4 doses in 24 hours 1, 2

Medication Components and Mechanism of Action

  • Polyhist Forte D contains diphenhydramine, a first-generation antihistamine that blocks histamine H1 receptors to reduce sneezing, itching, and rhinorrhea, and pseudoephedrine, an oral decongestant that reduces nasal congestion through vasoconstriction 2, 3
  • The combination of diphenhydramine and pseudoephedrine provides complementary effects, with diphenhydramine addressing histamine-mediated symptoms and pseudoephedrine targeting nasal congestion 1, 3, 4

Specific Dosing Guidelines

  • For adults and children over 12 years, the recommended dosage is 25-50 mg of diphenhydramine with 60-120 mg of pseudoephedrine every 4-6 hours, with a maximum of 4 doses in 24 hours 2, 5
  • For children 6-12 years, reduced doses (typically half the adult dose) should be used with caution, and consultation with a physician is recommended 6

Precautions and Contraindications

  • Patients with hypertension, cardiovascular or cerebrovascular disease, hyperthyroidism, closed-angle glaucoma, or bladder neck obstruction or prostatic hypertrophy should use Polyhist Forte D with caution, monitoring blood pressure and being aware of potential side effects 4, 7
  • Potential side effects of Polyhist Forte D include sedation and performance impairment, anticholinergic effects, insomnia, irritability, palpitations, and blood pressure elevation 1, 3, 4, 5

Clinical Pearls

  • First-generation antihistamines like diphenhydramine are preferred over newer generation antihistamines for treating cold symptoms due to their anticholinergic properties, according to the American College of Chest Physicians 7, 8
  • Avoid concurrent use of Polyhist Forte D with caffeine or other stimulants, as this may increase adverse effects, and consider intranasal corticosteroids for long-term treatment of allergic rhinitis 1, 4
