Fibromyalgia Management Guidelines

Evidence-Based Treatments

  • The American College of Rheumatology recommends FDA-approved medications, such as duloxetine, milnacipran, and pregabalin, for fibromyalgia management, which have demonstrated small to moderate improvements in pain, function, and quality of life 1, 2, 3
  • Tricyclic antidepressants, particularly amitriptyline, are recommended despite limited evidence of effectiveness, with the guideline society suggesting their use with caution 4
  • Specific anticonvulsants, such as pregabalin and gabapentin, have shown efficacy in reducing fibromyalgia pain, with a moderate strength of evidence 3, 4

Pharmacological Options

  • Tramadol is recommended for pain management in fibromyalgia with level Ib evidence, indicating a high strength of recommendation 5, 6, 7
  • Simple analgesics, such as paracetamol, and other weak opioids can be considered as adjunctive treatments, although their use should be carefully evaluated due to potential risks 5, 8

Non-Pharmacological Interventions

  • Heated pool treatment with or without exercise is effective for fibromyalgia, with level IIa evidence, indicating a moderate strength of recommendation 6, 9
  • Individually tailored exercise programs, including aerobic exercise and strength training, are beneficial for fibromyalgia management, with a moderate strength of evidence 6, 7
  • Cognitive behavioral therapy may benefit some patients with fibromyalgia, although the strength of evidence is limited 6, 9

Treatment Selection and Clinical Considerations

  • The guideline society recommends starting with FDA-approved medications, such as duloxetine, milnacipran, or pregabalin, and considering tricyclic antidepressants, such as amitriptyline, if not contraindicated 3, 4
  • Tramadol can be added for additional pain control if needed, although its use should be carefully evaluated due to potential risks 5, 6
  • Regular reassessment of treatment efficacy and side effects is essential to optimize management, with the guideline society recommending avoiding unintentionally continuing ineffective medications without proper evaluation 3
