Neuroendocrine Tumor Chemotherapy Regimens

Treatment Selection Based on Tumor Grade and Differentiation

  • The National Comprehensive Cancer Network recommends chemotherapy regimens for neuroendocrine tumors be selected based on tumor grade, differentiation, and proliferative activity, with poorly differentiated tumors requiring platinum-based regimens and well-differentiated pancreatic NETs responding to streptozotocin-based combinations 1, 2, 3

Chemotherapy Regimens for Poorly Differentiated Tumors

  • The National Comprehensive Cancer Network suggests using platinum-based chemotherapy following small cell lung cancer protocols for poorly differentiated or small cell neuroendocrine tumors 1, 3, 4, 5
  • Carboplatin (AUC = 6) + Etoposide (50-100 mg/day PO days 1-10) every 3 weeks is a recommended regimen for poorly differentiated neuroendocrine tumors 1, 4, 5
  • Alternative regimen: Cisplatin (100 mg/m²) + Etoposide for poorly differentiated neuroendocrine tumors 3
  • Paclitaxel (200 mg/m² IV) + Carboplatin (AUC = 6) + Etoposide (50 mg/day PO alternating with 100 mg/day) every 3 weeks achieved 53% response rate with median survival of 14.5 months in poorly differentiated neuroendocrine carcinomas of unknown primary 3

Chemotherapy Regimens for Well-Differentiated Pancreatic Neuroendocrine Tumors

  • The European Society for Medical Oncology recommends streptozotocin-based combinations as the standard cytotoxic approach for advanced pancreatic NETs 2
  • Streptozotocin + 5-FU + Doxorubicin achieved response rates of 40-60% with median survival of 2 years in well-differentiated pancreatic NETs 2
  • 5-FU + Dacarbazine + Epirubicin (standard or intensified dosage) achieved 20-30% response rate with median response duration of 38 months in well-differentiated pancreatic NETs 2
  • Lomustine + 5-FU showed similar response pattern to above regimens in well-differentiated pancreatic NETs 2

Patient Selection Criteria

  • The National Comprehensive Cancer Network recommends considering chemotherapy in symptomatic patients with ECOG performance status 1-2 and aggressive disease characteristics 1, 4, 5
  • Asymptomatic patients (PS 0) with aggressive cancer characteristics should also be considered for chemotherapy 1

Monitoring During Treatment

  • The European Society for Medical Oncology recommends follow-up every 3 months during active treatment to assess response in patients receiving cytotoxic chemotherapy 2
  • Include specific biochemical markers (chromogranin A, 5-HIAA) depending on tumor functionality, and imaging with CT or MRI every 6 months 2, 7

Adjunctive Surgical Considerations

  • The European Society for Medical Oncology suggests considering prophylactic cholecystectomy when performing abdominal surgery in patients likely to receive long-term somatostatin analogue therapy to prevent gallstone formation 2, 8
