Hypertension Management with Nicardipine and Nitroglycerin

Initial Dosing and Titration

  • The American College of Cardiology recommends starting nicardipine at 5 mg/hr and titrating by 2.5 mg/hr every 5-15 minutes to a maximum of 15 mg/hr for hypertensive emergencies requiring IV therapy 1, 2
  • The American Heart Association suggests starting nitroglycerin at 5 mcg/min and increasing by 5 mcg/min every 3-5 minutes to a maximum of 20 mcg/min 1, 2, 5

Clinical Context-Specific Selection

  • The European Society of Cardiology recommends choosing nicardipine for acute renal failure, eclampsia or preeclampsia, perioperative hypertension, and acute sympathetic discharge states 6, 8
  • The American College of Cardiology recommends choosing nitroglycerin for acute coronary syndrome, acute cardiogenic pulmonary edema, and hypertensive heart failure 3, 6

Blood Pressure Reduction Targets

  • The American Heart Association recommends reducing SBP by no more than 25% within the first hour, then aiming for 160/100 mmHg within the next 2-6 hours if stable, and cautiously normalizing over the following 24-48 hours for patients without compelling conditions 1, 2, 5
  • The European Society of Cardiology recommends reducing SBP to <120 mmHg within the first hour for aortic dissection, and to <140 mmHg within the first hour for severe preeclampsia/eclampsia or pheochromocytoma 1, 2

Critical Safety Considerations

  • The American College of Cardiology warns to monitor for tachyphylaxis with extended nitroglycerin use 1, 2
  • The European Heart Journal recommends considering additional beta-blockade if tachycardia develops with nitroglycerin 3
