Management of Diarrhea with Lomotil

Primary Indications and Contraindications

  • The Infectious Diseases Society of America recommends that Lomotil should not be given to children <18 years with acute diarrhea 1, 2
  • Lomotil is conditionally recommended for acute noninfectious diarrhea in critically ill adults, and is considered a second-line option after loperamide 3, 4
  • The American Society of Clinical Oncology recommends loperamide as the preferred first-line antimotility agent for cancer treatment-induced diarrhea, with Lomotil as an alternative after loperamide failure 3, 4

Critical Contraindications

  • The Centers for Disease Control and Prevention recommends avoiding Lomotil in patients with inflammatory diarrhea or diarrhea with fever, due to the risk of toxic megacolon 1, 2
  • The Infectious Diseases Society of America recommends avoiding antimotility agents, including Lomotil, in patients with bloody diarrhea or suspected dysentery 1, 5
  • The European Society for Medical Oncology recommends careful risk-benefit assessment in neutropenic patients, as overdosage of antimotility agents may lead to iatrogenic ileus with increased risk of bacteremia 3

Dosing and Duration

  • No cited facts are available for this section

Clinical Algorithm

  • The Journal of Clinical Oncology recommends obtaining stool cultures before starting antimotility therapy, especially if fever is present 4, 6
  • The European Society of Gastrointestinal Endoscopy recommends starting loperamide (not Lomotil) while awaiting results in immunocompetent adults 7
  • The Infectious Diseases Society of America recommends loperamide as the preferred first-line agent for most cases of noninfectious diarrhea 1, 3, 4

Important Caveats

  • The Infectious Diseases Society of America recommends that antimotility agents are not a substitute for fluid and electrolyte therapy, and rehydration must be addressed first 1, 2
  • The American Society of Clinical Oncology recommends octreotide as first-line therapy instead of antimotility agents, including Lomotil, in cancer patients with severe (grade 3-4) diarrhea 4

Antimotility Agent Use in Diarrhea

Contraindications and Precautions

  • The Infectious Diseases Society of America explicitly recommends against antimotility drugs, including Lomotil, in all children and adolescents with acute diarrhea, due to the risk of worsening outcomes 8
